This waiver is required for anyone participating in candle making or any craft using a hot glue tool. Participants must be 18 years or older. Please complete this before your scheduled time.

By signing below, I acknowledge that I am voluntarily participating in a candle-making and/or hot glue craft activity at Shadow Botanical Art Studio, located at 1 W Magnolia St, Arcadia, FL 34266.
I understand that this activity involves the use of open flame, melted wax, and/or a hot glue gun, all of which reach temperatures that can cause burns or injury if mishandled. I agree to follow all instructions given by studio staff.
I confirm that I am at least 18 years of age. I understand that this activity is not available to minors, with or without adult supervision, due to the burn risk involved.
I release Shadow Botanical Art Studio, its owners, staff, and Boarhart Dice LLC from any liability for injury, loss, or damage arising from my participation in this activity, except in cases of gross negligence.
